Alberta Health Services’ focus is creating an environment where you are able to live at home independently for as long as possible. While Home Care services are most commonly delivered in your home, flexibility exists to deliver services in a variety of other settings

Case Manager

All Home Care Clients will be assigned to a Home Care Case Manager. Your Home Care Case Manager will identify your needs and link you with the most appropriate available home and community support services.

  • Together with your Case Manager, you will create a personal care plan to support your wellness and independence.
  • Your Home Care Case Manager has access to a team of health-care professionals to assist in meeting the goals of your care plan – including your family physician, physiotherapists, occupational therapists, nurses and more.
