Going Home

Purpose of This Area

Your own planning for going home should continue once you arrive on the Patient Care Unit after surgery. Your discharge date is a medicalGoing Home decision made by your surgeon, in discussion with you and your health care team. Please advise your care team if you have concerns about your discharge and/or your ability to manage at home. Please ask about activities that may require help, so planning can happen before the day you go home. Often it is beneficial to have somebody to assist with cooking, housekeeping and sometimes bathing. Being an active participant in your preparation for home will ensure a successful transition.

What Happens to Me

Before discharge, your care team will provide you with instructions so you will be able to care for yourself at home. These may include information on medication, diet, activity, wound care, and follow up appointments as required. Information regarding symptoms to watch out for should also be included. Asking questions will help you understand the information given. If you have any problems or questions after your return home, contact your surgeon or family doctor.

Please arrange a ride home before your discharge date. Discharge time is usually 10:00 a.m., but you may be discharged at any time during the day or early evening. If your ride cannot pick you up by 1000 a.m., you may be asked to wait in the Discharge Lounge area.

If you have any questions, concerns or bouquets about your care, please speak with your nurse, the charge nurse, the Assistant or the Patient Care Manager.
