Alberta Healthy Living Program-Diabetes Education and Management

Supports people with diabetes (type 1, type 2,  Insulin pumps and gestational) learn skills and make lifestyle changes that can help them manage the disease. Services include:


  • one-to-one counselling
  • 12 week supervised exercise program
  • education sessions, group classes, and workshops (pre-diabetes, gestational, lifestyle and disease, Better Choices, Better Health™)
  • programs to help with exercise, quitting smoking, and managing weight
  • appointments with doctors who specialize in internal medicine
  • help starting insulin and adjusting insulin
  • blood pressure checking
  • group visits for people who have the same medical issues (e.g., insulin starts, gestational)
  • Telehealth services for rural sites

Referral is required to access internist services. Internist and pediatric services are available at the Medicine Hat location only


There is one referral form for the Living Healthy program which provides a number of service requests including diabetes clinic.
