Diabetes Education and Management Clinic

Provides education and care to people with diabetes in an outpatient setting so that they may learn self-management skills and make lifestyle changes in order to:

  • effectively manage their diabetes
  • prevent or delay complications
  • achieve self-defined optimal health
  • improve quality of life

Services offered may include:

  • individual counselling
  • group educational sessions (classes)
  • gestational diabetes program
  • outpatient commencement of insulin and insulin adjustment
  • one-day diabetes education sessions
  • self-management workshops (Better Choices, Better Health™)
  • prediabetes education
  • exercise programs
  • smoking cessation
  • weight management
  • lifestyle and disease specific education
  • internal medicine consultation
  • eye screening clincs
  • 24-hour ambulatory blood pressure clinic
  • group medical visits (e.g.) insulin starts, gestational
  • outreach clinics including telehealth services to rural sites
  • insulin pump therapy

Clients access services through referral by physician or health care provider. Client self-referrals are accepted. Referrals are triaged and determination of need is prioritized to ensure the right care is provided by the right provider at the right time.

There is one referral form for the Living Healthy program which provides a number of service requests including diabetes clinic. All referrals are triaged and prioritized.
