Inpatient Geriatric Consultation Service
This service provides:
- medical and care planning consultation for medical and surgical inpatients by a geriatrician and clinical nurse specialist;
- assistance with management of complex behaviour and medical problems of the frail older adult.
The consultation service is available to older adults in Calgary's acute care hospitals. They may be patients who are not progressing as expected toward discharge, or who have complex medical problems.
The Inpatient Geriatric Consultation Service is comprised of physicians, nurses and pharmacists who are specialists in the health needs of individuals over the age of 65. This service is available at all three of Calgary's acute care hospitals.
Individuals seen for assessment through this service are either:
- Those who require multifaceted assessment focused on medical and cognitive status, and functional and social capabilities or
- Those who potentially require admission to the inpatient Geriatric Assessment and Rehabilitation Program (GARP) before returning home.
As part of the consultation, recommendations are made for the current management (medical and/or nursing care) and may involve discharge planning suggestions (e.g. referral to Seniors' Health Community Services such as: Day Hospital; out-patient Seniors' Health Clinics.
