Shared Mental Health Care
This Program helps family physicians to assess, treat, and manage their patients' mental and/or behavioural health concerns through regular consultation provided at the physician's practice. It involves collaborative relationships between primary care providers (family physicians) and mental health consultants (psychiatric nurses, clinical social workers, psychologists, psychiatrists). Services include assessment, intervention, and educational activities and occur during regular appointments.
Shared Care is the traditional service whereby the family physician is partnered with a Mental Health Consultant who is required to be in the room during the patient’s sessions. These physicians also have access to a psychiatrist.
Behavioural health consultants are mental health consultants integrated into the primary care clinic where they can take referrals from the physicians for any mental or behavioural health concerns in their patients. Appointments often involve only the patient and the behavioural health consultant, and are shorter in length than a shared care session.
Consultation can also occur without patients in the form of discussions regarding assessment, treatment, mental health resources, and referral issues. The family physician retains primary mental health care responsibility for patients.
