Specialized Geriatric Services (SGS) includes both the Seniors Health Clinic and the Acute Care of the Elderly or ACE Team.
Note: This service does not replace Seniors Mental Health whose focal area is behavioral and psychiatric illness.

Referral needed
Assessment if referrals is completed by interdisciplinary team in consultation with Internal Medicine Specialists. Patients in all clinics should have a family physician who is able and willing to collaborate in the care and management of clinic patients, and to resume care when patients are discharged from the clinic. Referrals to the Seniors Health Clinic are made by a family physician who is willing to resume care following assessment and recommendations made in the clinic. Inpatient Service: Referrals to the Acute Care of the Elderly (ACE) Team are also made by a family physician who is willing to resume care of hospital inpatients following assessment and recommendations. Special referral criteria is available on request.
