Social Work
The Social Worker, under the direct supervision of Community Health Services Manager, functions in a consultative role to provide support to the community and facility programs.
- Refer to and liaise with appropriate community and government agencies.
- Collaborate with other health professionals (discharge planning, family conferences).
- Provide supportive counselling and crisis intervention services.
- Provide information/education on relevant resources and legislation (e.g., Powers of Attorney, Guardianship, Child Welfare and Protection for Persons in Care).
