Referral needed
The referral process is changing Provincially and all details will be communicated once the processes are decided upon. Please contact the Bariatric Specialty Clinic if more information is needed. Referral criteria not supplied by the Family Physician's office will not be accepted. If your patient is outside of this referral criteria the physician or nurse practitioner must contact one of the surgeons directly to discuss. All referral letters faxed to the specialty clinic should include referring practitioner, up-to date demographic information, medical and surgical history, height, weight, medication list.
