Continuing Care Services

Our continuing care services are designed for those who can no longer be cared for at home or in an assisted living facility due to illness, injury, frailty, or other limitations. Our nursing homes in the Canada employ licensed and certified nurses, therapists and certified or specially trained health care aides. All nursing homes are either licensed or otherwise operate under contract with governing authorities and in accordance with legislation. The facilities provide long-term care as prescribed by each resident's attending physician in collaboration with an interdisciplinary care team.

Continuing Care in the region has a single access point through which their choices and needs are respected and the services available are matched fairly and appropriately. A physician, caregiver, or family member may request an assessment for Continuing Care at any time. Community (Home) Care nurses are responsible for completing assessments using a standard form (the Alberta Assessment and Placement Instrument). When an applicant is approved for Continuing Care, their name is placed on a Regional waiting list until an appropriate placement becomes available.

Applications for Continuing Care are considered by a Placement Committee. The committee's decisions are based on client needs and available resources. The Access Coordinator will inform applicants, in writing, of the Committee's decision.

The level of services offered by each facility can vary. It may be necessary for a client to accept a temporary placement until a space becomes available in the facility of their choice. Service fees for Continuing Care vary according to the type of accommodation and services required. Fees are regulated and updated periodically by Alberta Health and Wellness.
