Social Work

Provides support to community and facility programs.

Services may include:

  • refer and liaise with appropriate community and government agencies
  • collaborate with other healthcare providers (discharge planning, family conferences)
  • provide counselling and crisis intervention
  • provide information/education on important resources and legislation (e.g., Personal Directives, Guardianship, Trusteeship, Child Welfare and Protection for Persons in Care)
