Preschool Assessment Service - Children - Glenrose Rehabilitation Hospital
The 1 to 3 day assessment:
- can help families/caregivers understand and learn more about the child's developmental disability/disabilities
- directs the family/caregiver to an appropriate program
The family is given a brief summary of the findings, diagnosis, and recommendations on the day of assessment.
A detailed report is mailed to the family and doctor after the assessment.
PAS (Preschool Assessment Service) doesn't do assessments when the purpose is only to access treatment funding, determine school or adoption placements, or monitor progress.

Form available from PAS Clerk (Phone 780-735-7906) Form must be signed by a doctor/nurse practitioner and parent/legal guardian. Completed forms can be faxed back to 780-735-7976 or mailed to Admitting at the Glenrose Rehabilitation Hospital (address on the referral form).
