Preschool Assessment Service - Children - Glenrose Rehabilitation Hospital
The 1 to 3 day assessment:
- can help families/caregivers understand and learn more about the child's developmental disability/disabilities
- directs the family/caregiver to an appropriate program
The family is given a brief summary of the findings, diagnosis, and recommendations on the day of assessment.
A detailed report is mailed to the family and doctor after the assessment.
PAS (Preschool Assessment Service) doesn't do assessments when the purpose is only to access treatment funding, determine school or adoption placements, or monitor progress.

The child's Physician or Nurse Practioner must complete the Glenrose Pediatric Referral Form with the parent or caregiver to support shared care and onging medical follow-up for any recommendations.
Recent information from the school, community program, or community therapists are important to help us meet the child's needs. Please include these reports with the referral.
