This service is for adults who are ready to leave acute care but need more support before they’re discharged home or to another facility. Along with more assessment, time to recover, and/or rehabilitation, they also have time to find resources for funding, guardianship, etc. if they need it.

A referral is needed to access this service. Call for more information.
The individual is assessed by their doctor, Care Coordinator, and hospital Care Team to see if this is the best service for them. Arrangements are made once a bed is available for the service they need. Community Bed Hub will decide the most appropriate available bed.
There is no charge when the Transition Care program is an extension of the care that would be required in a hospital. However, once a client/patient is assessed and approved for placement to a continuing care option, charges will commencce.
