Case Management Coordination - Elizabeth Settlement Community Health Services
Referrals for an assessment may be made by a physician, patient and or family. The assessment form is filled in by a Home Care nurse and forwarded to the area case management coordinator. The case management coordinator reviews the information obtained & puts resources in place, assists with the transfer in accommodation: a lodge, nursing home or assists the client to stay home as long as possible or while safety allows.
