- Tuesday 8:00 am - 4:00 pm
- Wednesday 8:00 am - 4:00 pm
- Thursday 8:00 am - 4:00 pm
- Friday 8:00 am - 4:00 pm
Routine requests for health information are processed during the week and during regular office hours only.
Fees
$25. Up to 25 pages of copied information. Additional $0.25 thereafter. Payment must be received prior to start of prcessing request. In accordance with Health Information Act and PCH Policy A-1X-625.
