Health Information / Access and Disclosure - Cochrane Community Health Centre
Upon written request/consent, medical information can be released to individuals, other service providers, lawyers or any other authorized party in accordance with the Health Information Act.

Main entrance is in the back of the building. Washrooms are tight but could get a wheelchair and does have grab bar to assist client.

- Tuesday 8:00 am - 4:15 pm
- Wednesday 8:00 am - 4:15 pm
- Thursday 8:00 am - 4:15 pm
- Friday 8:00 am - 4:15 pm
Fees
$25. Up to 25 pages of copied information. Additional $0.25 thereafter. Payment must be received prior to start of prcessing request. In accordance with Health Information Act and PCH Policy A-1X-625.
