Calgary Headache Assessment & Management Program

(Previous names: Calgary Headache Assessment Program, Migraine/Headache Services)

In addition to medical management, the Calgary Headache Assessment & Management Program (CHAMP) offers workshops, lectures, and an education session to empower patients with knowledge, and to teach skills in headache self-management.

Available to

People aged 16 - 70 years who experience approximately 8 - 15 days of headache per month.

Referral needed

Physician to fax (403-956-3497) Department of Medicine, Calgary Zone, Central Access and Triage referral form and the CHAMP "Information Needed to Enable Triage" form.
