Home Care

Clients receive an in-home assessment by a Community Care Coordinator. The coordinator works with the client and the family to determine an appropriate care plan which may include:

  • short-term, long-term and palliative professional care and support services to persons of all ages and any diagnosis;
  • assistance to people requiring care who are being discharged from hospital or wishing to remain in their own homes, who require professional health and supportive care services;
  • registered nurse and licenced practical nurse care;
  • physiotherapy or occupational therapy;
  • speech language pathology;
  • respiratory care;
  • social worker consultation;
  • nutrition counselling;
  • personal care;
  • homemaking.

Home Care Services may be provided directly by Alberta Health Services staff or through contracts with community agencies.

Referral by family physician, family member or other third party, or self referral.  For referrals in the Calgary Zone, call Community Care Access at 403-943-1920 for further information.

Fees

Some clients are charged a small fee for homemaking service based on income. Non-Alberta residents will have to pay full cost for services.

Dietitians, Home Support Workers (homemaking or personal care), Licenced Practical Nurses, Occupational Therapists, Physical Therapists, Registered Nurses, Respiratory Therapists, Social Workers, Speech Language Pathologists, Volunteers
