Continuing Care Counselling

The services are offered to clients in Homecare, acute care and continuing care centres.

Clients, their family and / or caregivers are offered counselling services and are linked to other health care professionals and agencies, as needed. The counsellors provide information on housing, Personal Directives, Guardianship, Trusteeship, Powers of Attorney, etc. to clients, families, and staff.  For example, they can facilitate problem-solving and streghten coping skills, promote understanding, ease emotional distress and provide short-term counselling on issues around admission and discharge to facility, transition to other levels of care, rehabilitation, aging, adjustment, crises, psychosocial and bereavement support. 

Service is offered through home visits, lodges, Acute Care, Continuing Care Centres and Supportive Living residences.

Available to

Clients in the Continuing Care System

Referral needed

Counsellors receive referrals from Home Care Case Coordinators, facility team leaders, intake workers, and Hospital Discharge Planners. A Referral Consultation Home Care form is preferred.

Service providers

This is a multidisciplinary team consisting of Registered Nurses,Registered Psychiatric Nurses,Social Workers and other professionals.
