Supportive Living Level 3

Supportive Living offers assistance with daily living and health care needs over a 24 hour period. Service includes scheduled and unscheduled personal care, housing, meals, housekeeping, linen laundry service, recreation programs and 24 hour safety and emergency response services. Professional services are provided by Home Care on a scheduled basis.

Required age 18 years and over.

Applicants are listed on a regional waitlist for admission when space available.

Referral needed

The Home Care office receives referrals via telephone call, email, fax or in person from individuals, families, health care providers, and physicians. Details are recorded on the Community Care Intake data form. The Home Care RN Case Coordinator then sets up a date with the client / family to complete an assessment using the current Home Care assessment tool and submits the application to the Continuing Care Placement Office. Applicants who qualify for this level of care must then contact the Supportive Living home of choice for completion of housing and financial assessment as applicable.

Fees

Housing fee applicable and varies between sites. Supportive living targets individuals who are unable to manage in their own home or lodge setting, and do not qualify for continuing care placement. Fees may apply.
