About Us

This program offers supportive palliative care to terminally ill patients and their families. The team is comprised of palliative care physicians, nurses, social workers, spiritual care providers and other allied health care professionals.

Services Offered

Services provided may include:

  • Symptom management (e.g. pain, delirium, nausea etc.)
  • Maintenance of optimal physical and cognitive function
  • Psychological and spiritual support
  • Patient/family education regarding disease progression and the end of life
  • Prognosis and goals of care discussions and designations
  • Discharge planning and coordination of palliative care services
  • Transitioning to hospice
  • Grief counselling

Referrals / Consults

Palliative Care Consultation

Your physician or Home Care Case Manager can request a Palliative Care Consult using one of the forms below:

  • Urban Palliative Care Consult Request form
  • Rural Palliative Care Consult Request form
  • SL/LTC Palliative Care Consult Request form

Home Care

If you are not a Home Care client, contact Community Care Access Calgary Zone.


Only a Palliative Care Consultant or Palliative Home Care Case Manager can refer to Calgary Zone hospices. If you think hospice may be right for you or your loved one, talk to your physician or Home Care Case Manager about a Palliative Care Consult. Questions?
