Heart Function Clinic

The Heart Function Clinic provides specialized expertise in assessment, therapy, monitoring and education of heart failure through an interdisciplinary team of professionals. They improve quality of life and the ability for clients to self -manage their condition through education, informed decision making and coordination of care.

The Heart Function Clinic helps patients and their families meet the challenge of heart failure care through a supportive relationship with a multidisciplinary team of health care professionals. Important services include:

  • initial and ongoing evaluation of condition
  • improved coordination of care
  • empowering patients with confidence, skills and ability to effectively self - manage HF
  • providing individualized patient education
  • optimization of heart failure care including medication use and dose adjustments
  • promotion of positive life style changes
  • patient / family support
  • referrals to specialists and other programs

The clinic encourages self - management with regular assessments, evaluation and follow - up with team members.
