When publishing for paths, change the title name to reflect the path and the role variable:

Duplicate Record

 

A duplicate record results when more than one RHRN has been created for a patient in Clinibase. If a duplicate exists, it could impact patient care when important information only appears on one of the records. If you think there is a duplicate record, email Data Integrity cal.diu@ahs.ca indicating both RHRN's.

Overlays

 

An overlay occurs when the wrong patient is identified and used to create an encounter. This is similar to writing on a wrong chart but harder to identify and rectify when it is done electronically. For example; Shield Flake's record is identified and used to create an encounter instead of the correct patient, Shiver Flake. If an overlay is suspected or identified, contact the Solution Centre immediately.


Errors with validating PIEM information can impact patient care and have significant negative effects.

 

The quality of patient care is jeopardized when a patient is not identified correctly in Clinibase. Two of these errors are:

Information entered in Clinibase impacts other electronic systems or downstreams. If incorrect information is sent to the laboratory, pharmacy, or diagnostic imaging department, significant issues can arise. If demographics or clinical information is wrong on the record, patient care will be compromised. Staff workload increases significantly to correct these errors.

 

Always exit out of all windows prior to searching for another patient to decrease the risk of errors.
