When the form opens, several fields will have information filled in for you, however you can edit fields that are white.

 

Once all the information is accurate on the form, click OK to enter the documentation into the system.

 

Click each number to learn about different sections of the Medication Administration Form.

 

Administered At

The current date and time are automatically populated when this form opens. Edit these fields as necessary to ensure that the actual administration time is reflected.

 

Task Information

This information comes from the order. It cannot be altered. If this information is incorrect, the order must be changed.

 

Documentation Details

Information from the order will auto-populate to the appropriate fields. If necessary, edit the information to reflect what was actually administered.

 

Co-Signature

x

If a co-signer is needed, click  and search for the clinician's name. Notice this is the same window layout that we used for the Mark as Done by Other function.

 

 will be displayed in the cell indicating that a signature is required. The initials of the person that administered the medication is displayed, not the co-signer's. The co-signer will see a red flag in the To Sign column on the Patient List tab for this patient.

 

 

Additional Information

Minimal charting should be entered in the Additional Information field. This does not replace clinical documentation.

An example of optional documentation for a PRN tylenol administration might be “elevated temperature”.

However, the actual vital signs would be entered on the flowsheet which can be accessed using the Flowsheet button.
