Specimen Collection Requirements: | A stillbirth collection kit is located at CRH Labor and Delivery as well as each SZ West Rural Hospital Site. Each kit contains instructions, sterile supplies and collection materials for both cord blood and placental tissue investigation. Refer to “Stillbirths/Product of Conception/live birth" Upon delivery of the stillborn physician must indicate the tests required on the Stillborn Examination and Investigation Form. Doctor must check off testing needed on the stillbirth form The following completed form/requistions MUST accompany the specimen • Stillborn Examination and Investigation Form (one copy) (HS001-128) can be obtained from Data Group If autopsy is requested refer to: Autopsy, fetal (<20 weeks) or Autopsy, fetal (>20 weeks) • Routine Anatomic Pathology Consultation Request (Requisition) (indicate what testing/sampling has already been performed on the specimen if sample is being sent for Histology • Requisition for Constitutional Cytogenetics Services - Karyotype, FISH and RAD (Calgary Laboratory) (full patient history required) • Serology and Molecular Testing Requisition (Cross-site) | ||
