Specimen Collection Requirements:	For those sites that refer to DynaLIFE Medical Labs, see DynaLIFE Medical Labs or contact DynaLIFE Specimen Control at 1-800-661-9876. Midstream: The following is explained to the patient: a.Wash hands b.Wash genital area using the antiseptic towelette. i. Males; expose the glans and wipe with one downward stroke ii. Females; separate the labia and wipe with one downward stroke. DO NOT wipe up c.Begin voiding into toilet. d.Place the BD blue-top sterile container into the urine stream and allow it to fill. e.Remove the container before the flow is finished. f.Place the lid securely on the container. g.Place container where instructed. Transfer IMMEDIATELY to the grey-top preservative tube, using the vacutainer device in the blue-top lid. If the urine is collected at home by the patient, the sample should be collected in a pink or orange-top sterile container instead of the blue-top container (which contains a sharp, transfer device in its lid). After this is delivered to the collection site, a healthcare worker should transfer urine into the grey-top preservative tube using a transfer straw. This MUST be done IMMEDIATELY for accurate results. Specify “Midstream urine” In/out catheter urines from children < 5yrs age: Clean the patient's urethral opening (and in females, the vaginal vestibule) with soap and water. Wash hands well with soap and water. Dry well. Wear sterile gloves. Using a sterile technique, pass a catheter into the bladder. Discard the initial 15-30 ml of urine. Collect the next 30 ml in the BD Vacutainer (blue-top) urine collection cup. Close the lid. Transfer urine from the collection device IMMEDIATELY into the grey top preservative tube using the vacutainer device in the lid. The latter can be stored and transported at room temperature. Specify “In/Out catheter urine” For cystoscopy, suprapubic aspirate, nephrostomy and urostomy: Place 20-30mls urine into the BD blue-top, sterile container. Transfer IMMEDIATELY to the grey-top preservative tube, using the vacutainer device in the blue-top lid. Specify collection method Pediatric bag specimen: Cleanse the genital area using an antiseptic towelette. Be sure to dry the skin thoroughly to assure proper adhesion of the collector Remove the backing paper from one pediatric urine collector. Place collector against perineum (carefully, so as to avoid possibility of fecal contamination). Work upwards and smooth the adhesive against the skin. Avoid wrinkles which can create leakage. Excess of bag should be towards feet. Check collector periodically (10-15 minutes) so that collector can be removed as soon as a specimen is voided. When the specimen has been obtained, gently peel the adhesive patch from the skin, taking care not to spill the contents. Transfer the specimen to a properly labelled BD blue-top sterile urine container. Transfer IMMEDIATELY to the grey top preservative tube using the vacutainer device in the blue-top lid. Post prostatic massage: IDEALLY, FOUR SPECIMENS SHOULD BE COLLECTED FOR THE INVESTIGATION OF PROSTATITIS (4-GLASS STAMEY TEST). The first 5-10mls of voided urine (VB1) is the urethral sample. The second (midstream) sample of approximately 30 ml (VB2) is the bladder sample. The patient must stop voiding before the bladder is empty The prostate is then massaged for about one minute and the expressed prostatic secretions (EPS, prostatic sample) collected. The patient then voids the first 5-10 ml of urine (VB3), the prostatic sample, immediately after prostatic massage. All specimens should be submitted in BD grey top vacutainers. Label each container accurately, i.e. VB1, VB2, EPS and VB3, each ordered separately. In/Out (Straight) Catheter Urine Collection: Preferably, the specimen should be collected 2-4 hours after the last void to increase the chances of detecting the pathogen. Clean the patient's urethral opening (and in females, the vaginal vestibule) with soap and water. Wash hands well with soap and water. Dry well. Wear sterile gloves. Using a sterile technique, pass a catheter into the bladder. Discard the initial 15-30 ml of urine. Collect the next 30 ml in the BD Vacutainer (blue-top) urine collection cup. Close the lid. Transfer IMMEDIATELY to the grey-top preservative tube, using the vacutainer device in the blue-top lid. Specify “In/Out catheter urine” Indwelling Catheter Urine Collections: • If the catheter has been in place for <14d, urine can be collected from the existing catheter. • If the catheter has been in place for >/=14d, the catheter should be removed and either an MSU collected, or a new catheter replaced BEFORE collecting the specimen from the catheter. This ensures an accurate culture result and also helps the patient to recover. Urine specimens should be collected by a trained caregiver, using aseptic technique. Wash hands well with soap and water. Dry well. Wear sterile gloves. Urine should always be collected from the side port (NOT from the collection bag or catheter-tube junction) Clean the side port with an alcohol sponge. Using needle and syringe puncture the port diaphragm and aspirate urine. Place the aspirated urine (at least 30 mls) in the BD vacutainer (blue-top) urine collection cup. Transfer urine from the collection device IMMEDIATELY into the preservative-containing tube using the vacutainer device in the blue top lid. The latter can be stored and transported at room temperature. Alternatively, urine can be aspirated from the side port using a vacutainer device which is inserted into the cleaned side port, and the grey-top preservative tube directly inserted into the device. The tube is held in position till no more urine enters the tube, after which both vacutainer device and tube are removed. Specify Indwelling catheter urine
Processing:	Unacceptable specimens: Unpreserved specimens > 2 hours old, not refrigerated or received on ice or more than 24 hours on ice or refrigerated Preserved urines (BD urine culture grey top tube) can be kept at room temperature after collection, but results may be affected if specimen not received at the lab within 48 hours. Urine should be transferred in to the preservative tube IMMEDIATELY after collection for accurate results.
