Specimen Collection Requirements: | If the placenta is associated with a stillbirth please complete an Autopsy Consent and Consultation Request form for the stillborn fetus. The following instructions apply to the placenta: Label the container with the mother's information. Calgary Zone: Keep refrigerated until it can be sent fresh. Edmonton and Central Zones: Send in 10% NB formalin. If formalin unavailable, keep refrigerated until it can be sent fresh. North Zone: Send in 10% NB formalin. If formalin is not available, keep refrigerated until it can be sent fresh. South Zone: Complete a Routine Anatomic Pathology Consultation Request.(Requisition). See "Test Resources" for link.

If additional specimens are sent, a Routine Anatomical Pathology Requisition is required. If Cytogenetic testing is requested, complete an Genetics and Genomics Cytogenetics Analysis, Constitutional Requisition.
