Clinical Indications: | Urine cytology is indicated to diagnose high grade urothelial carcinoma and should only by a urologist or on the advice of a urologist. “Urine cytology recommended by urologist” should be indicated on the requisition or order when requesting urine cytology. Urine cytology specimens from community clinicians that have not been recommended by a urologist will be canceled in Epic. Urine cytology requested by urologist must be accompanied with clinical history. Urine cytology requests should be submitted on a non-gynecological cytology requisition. Requests on the advice of urologist must be accompanied by one of the following indications in the clinical history:

Specimen Collection Requirements: | Random urine, preferably not the first morning specimen. Containers must be securely tightened to avoid leakage. Place collected specimen in a sealed biohazard bag. A requisition must accompany all urine cytology requests, place in outside pouch. If tests other than Cytopathology are required, a requisition must be completed and accompany the specimen. | ||

Additional Test Information: | Early morning voided specimens are not suitable for Cytology because the cells in these specimens are usually too degenerate for cytologic examination.
Catheterized bladder urine is not recommended for Cytology. Catheters (and instruments) irritate and denude epithelial cells complicating cytologic interpretation. Please indicate whether the specimen was obtained through catheterization or following instrumentation, or if the patient has an in dwelling catheter. A waiting period of 1-2 weeks following the removal of a catheter is recommended before collecting a voided specimen. Indicate on the requisition the date of removal of the catheter. Urine specimens submitted in boric acid, denaturing medium, or that have been sitting in a drainage bag for a long period of time are unsuitable for cytologic examination. The chances of finding diagnostic cells increases with the volume of the submitted specimen. | ||
