Specimen Collection Requirements:

Place the biopsies, LEEP and endocervical curettage in separate formalin containers:

ONE SPECIMEN PER CONTAINER.

To avoid loss of nuclear detail through desiccation, do not put specimens on dry paper towels, papers sponges or cloths.

Label the containers with the exact specimen and site orientation (e.g. 6 o'clock, 9 o'clock).

All the specimens must be sent to the laboratory together.

Clarify the orientation on the Routine Anatomic Pathology Consultation Request (Requisition)

Containers must be securely tightened to avoid leakage.
Place collected specimen in a sealed biohazard bag.
Place requisition in outside pouch.
