Ordering Alert: | Record time of last dose on requisition or in electronic ordering system. For all other Vancomycin orders refer to Vancomycin Level, Other.
Collection Alert: | Collect within 30 minutes before next dose when at steady state. For adults: steady state is reached approximately 30 hours if normal renal function, before 4th dose if q12h dosing or before 5th dose if q8h dosing. For pediatrics: steady state is reached approximately 24 hours if normal renal function and after at least 2 maintenance doses. Best practice is to not draw from an intravenous line through which the drug has been administered as it can produce erroneous results and impact patient care. If best practice is not possible, prior consultation with the most responsible health practitioner is required.
