Pathogen: | Disorders of mitochondrial metabolism or unexplained refractory epilepsy. | ||||||||||||||||||||||
Clinical Indications: | Measurement of blood pyruvate, within the context of persistently elevated lactate, is used to distinguish between pyruvate dehydrogenase deficiency and other causes of congenital lactic acidosis. This test has no clinical utility outside of the context of elevated lactate. | ||||||||||||||||||||||
Test Includes: | Pyruvate and Lactate/Pyruvate Ratio are reported. | ||||||||||||||||||||||
Ordering Alert: | This test must be pre-booked with the Alberta Children’s Hospital (ACH) Biochemical Genetics Laboratory 403-955-7379 or with the University of Alberta Hospital Special Chemistry Laboratory 780-407-8491 at least 24 hours in advance. This test is considered specialty testing; this test will be cancelled if sufficient clinical justification is not provided. Test will NOT be performed if patient has NOT had previous evidence of elevated blood or CSF lactate level. This test must be ordered in conjunction with Lactate, Blood. Both tests (Pyruvate and Lactate) will be performed on the same specimen. Arterial or venous blood must be specified for correct reference range to apply to results. | ||||||||||||||||||||||
Collection Alert: | The patient must be fasting for at least 4 hours (if possible, discontinue IV dextrose) before collection.
