Clinical Indications: | This screen is a simple, rapid and sensitive method for the initial investigation of disorders resulting in increased excretion of thiol-containing metabolites (i.e., homocystinuria and cystinuria). This test should not be used to monitor known homocystinuria or cystinuria patients on treatment as this is a qualitative test; refer to Cystinuria Monitoring, Urine, 24 Hour. Specimen Collection Requirements: | Urine specimen MUST be kept FROZEN during entire collection and transport process. Refer to 24 Hour Urine General Collection - Patient Instructions for detailed instructions. Instruct patient to ensure the 24 hour urine collection container is placed into freezer immediately after each void throughout the 24 hour collection period. | ||||

Processing: | 1. Thaw 24hr urine specimen. 2. Measure total volume (TVOL). 3. Connect Care users, enter TVOL in Connect Care. All other users, write the TVOL amount on the requisition. 4. Aliquot minimum 1 mL of urine. 5. Write “TVOL #” on the aliquot label. 6. Freeze aliquot. | ||||
Transportation: | Transport frozen specimens on dry ice. Samples must not thaw during transport. | ||||
Method: | Colorimetric | ||||
Method and Interpretation of Results: | Increased urinary homocystine or cystine excretion is detected colorimetrically with the addition of sodium cyanide and sodium nitroprusside. Positive and negative controls are run with patient samples. A positive nitroprusside test suggesting the presence of either increased urine homocystine or cystine will result in reflex testing on this sample, volume permitting.
