Stability and Storage: | Calgary: Primary tube stable for 5 days in the fridge. Edmonton: Serum aliquot stable for 7 days in the fridge. Extended stability if frozen. Avoid multiple freeze thaw cycles. | ||||||
Processing: | Edmonton Zone: Aliquot serum into polypropylene or polystyrene tube and freeze. | ||||||

Method: | Indirect immunofluorescence assay IFA (ANCA slide) Multiplex bead immunoassay (anti-MPO, anti-PR3) | ||||||
Method and Interpretation of Results: | Positive results for antineutrophil cytoplasmic antibodies (ANCA) demonstrate 2 main patterns, cytoplasmic (C-ANCA) and perinuclear (P-ANCA). Negative ANCA results do not rule out a diagnosis of ANCA-associated vasculitis. Due to lack of diagnostic specificity, all positive ANCA results are confirmed using an antigen specific immunoassay using proteinase 3 ANCA for cytoplasmic ANCA and myeloperoxidase ANCA for perinuclear ANCA. Atypical ANCA patterns are infrequent comprising a mix of cytoplasmic and perinuclear fluorescence often with multiple antigen specificities and unclear clinical significance. Some ANA Patterns (homogenous, speckled, dense fined speckled DFS, and centromere) may be associated with false pANCA pattern. | ||||||
