Metanephrines, Urine, Random (Synonym: Met, Metanephrine, Normetanephrine, 3 Methoxytyramine, 3-MT, 3MT)

Clinical Indications:

Urine Metanephrines should be the initial test requested where pheochromocytoma is suspected.

Test Includes:

Metanephrine/Creatinine Ratio and Normetanephrine/Creatinine Ratio.
3-Methoxytyramine/Creatinine ratio is available on special request.

Ordering Alert:

For pediatric patients only. For patients 18 years old or greater, refer to Metanephrines, Urine, 24 Hour. Requests for adult random collections require pre-approval by Laboratory Medical or Scientific Staff.

Collection Alert:

During collection avoid bananas, caffeinated beverages, chocolate, vanilla, and peppers.

Stability and Storage:

Calgary Zone:
Store at refrigerated temperature.
Edmonton Zone:
If acidified, store at refrigerated temperature, otherwise store frozen.

Processing:

Mix well and aliquot.
Two aliquots are required for testing. One for Metanephrines and one for creatinine testing.

Edmonton Zone:
Adjust pH to less than 3.0 (pH paper) within 4 hours of collection by dropwise addition of 6 N HCl.
If specimen not acidified, freeze within 4 hours of collection.

Transportation:

Calgary Zone:
Transport at refrigerated temperature.
DSC Accession: Freeze specimen upon arrival at DSC.
Edmonton Zone:
If specimen has been acidified, transport refrigerated, otherwise transport frozen.

Additional Test Information:

False positive increases in urine and plasma metanephrines and catecholamines have been associated with the following drugs or classes of drugs: anti-depressants, anti-psychotics, some beta blockers, dihydropyridine calcium channel blockers, some anxiolytics, anti-Parkinson’s drugs, decongestants, phenoxybenzamine. Interpretation of results while on one of these agents should be interpreted with caution and put into clinical context.
