Senior's Day Programs

 Location: Melcor Centre

Provides support to older adults who want to continue living independently in their own homes.

The following 3 programs are intended to support older adults who want to remain independent in their own homes and include:

  • Bridges Day Program
  • Geriatric Community Rehabilitation
  • Adult Day Program

 

Adult Day Program

This program serves people who experience advanced challenges from dementia and live at home. We offer a safe environment for you to socialize and enjoy activities.

 

Attendance: Clients attend for six hours per day, up to twice per week.

 

Bridges

This program is for seniors who need support with maintaining their health and socializing. We help you stay independent for as long as possible, delaying or preventing a move to continuing care.

The program includes recreation, group exercises, health support and rehabilitation.

 

Attendance: Patients attend partial morning or afternoon sessions once or twice per week.

 

Geriatric Community Rehabilitation Program

This program serves people who have seen recent changes in their abilities after a surgery, illness or injury. Our multidisciplinary team assesses and treats patients.

 

Attendance: Patients attend 90-minute sessions twice per week, for up to 24 sessions.

Access
Service Providers May Include
dietitians, family practice doctors, geriatricians, health care aides, licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physical therapists (PTs), recreation therapists, registered nurses (RNs), therapy assistants, pharmacists
Eligibility

For seniors from Lethbridge and area who:

  • want to continue living in their homes, but have some risks
  • need 3 or more services from the program
  • are able to come to the program and other services
  • are able to move without a mechanical lift
  • are medically stable
Referral Needed

A written referral from your family doctor, home care RN or geriatrician to the Senior's Day Program is needed.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Once the referral is accepted, seniors are assessed to determine which of the 3 programs meets their needs.


Fees

Clients may need to pay a daily fee for services not covered by Alberta Health Care Insurance. For example, transportation or meals.
