Service provides assessment, diagnosis, treatment, and education for people who have been diagnosed with Hypertrophic Cardiomyopathy (thickened heart muscle).The Libin Cardiovascular Hypertrophic Cardiomyopathy Clinic (HCM) is a Nurse Clinician directed clinic with multidisciplinary support dedicated to the management of patients with suspected or confirmed HCM. Plan of care includes interval clinical assessment and follow-up by a cardiologist. Services provided may include: assessment, diagnosis, treatment, and education for people who have hypertrophic cardiomyopathy management of prescription medications pertaining to the patients HCM ongoing patient teaching lifestyle recommendations family screening teaching coordination of necessary specialist referrals
