Street Connect

Street Connect program provides client centered care to individuals who are houseless and living with high severity complex mental health needs within the City of Red Deer. The program will extend the continuum mental health and provide meaningful referrals to addictions programming.

The Street Connect team provides both outreach and in-reach services, trying to meet individuals where they are, to eliminate any barriers to accessing services. The mission of the Street Connect program is to provide client centered care to individuals who are houseless and living with high severity complex mental health needs within the City of Red Deer. The program will extend the continuum mental health and physical health service options for unhoused individuals with high severity complex mental health needs and provide meaningful referrals to addictions programming. Individuals who are appropriate for programming will work with the multidisciplinary team developing a treatment plan, goal setting, and working with clients for their mental health and medical needs. Warm hand offs and referrals are provided to community resources and link to more appropriate services for addiction and complex physical health needs. This service is provided by Recovery Alberta and Recovery Alberta contracted service providers.
