Rehabilitation and Restorative Care Intake
Provides single point of access intake to Rehabilitation and Restorative Care programs in the Central Zone. Programs under the umbrella of Central Intake include: Unit 35 at the Red Deer Hospital, Daysland Orthopedic and Active Rehabilitation (DOAR) program, Stroke and Geriatric Empowerment (SAGE) program in Two Hills and the Restorative Care programs in the Lacombe, Two Hills and Daysland Hospitals.Rehabilitation programs help a person become as independent as possible in everyday activities and enable participation in work, recreation and meaningful life roles. Illness and injury can result in a loss strength, endurance and functioning for patients. Rehabilitation programs are here to help you become stronger and better able to resume the tasks of living.
In a Rehabilitation Program you will work with a therapy team to regain skills or learn a new way to complete everyday tasks.
The hospital medical team will discuss the possibility of a rehabilitation program with you and then with your agreement will submit a referral form to Central Intake. Our team will review the referral, gather needed information, talk to you as the patient and your family about goals and match you with a program in Central Zone based on a variety of factors.
The programs in Central Zone not included in Central intake include: Halvor Jonson centre for Brain Injury, The Red Deer and Camrose Stroke Early Supported Discharge (SESD) program and Community Outpatient Rehabilitation programs.

This Central Intake service is for patients who are currently hospitalized and require additional rehabilitation support. We are not currently able to accept community referrals.
