Pediatric Heart Function

Provides outpatient ongoing follow-up care, management, and education for patients living with heart failure.

Services this program provides include:

• initial and ongoing assessment of heart failure
• providing individualized patient education to equip self-management of heart failure
• optimization of heart failure care including medication use and dose adjustments
• patient / family support
• ordering appropriate diagnostics / interventions as necessary
• referrals to specialists and other programs
