Complex Mental Health Rehabilitation Program

 Location: AgeCare SkyPointe

The Complex Mental Health Rehabilitation Program is primarily staffed by AHS staff, in collaboration with Protective Services, and AgeCare, providing 24 hours support to 32 complex mental health residents in our transitional rehabilitation program. Individualized care plans to address unique COPM goals. Length of stay is 6 months to 2 years.

This program offers:

  • client focused recovery care plans for skill development
  • specialized staff trained for managing clients with challenging behaviors, mental illness and / or concurrent disorders
  • psychiatrist and physician support
  • private, single occupancy suites with bathroom (clients pay accomodation fees to AgeCare)
  • enhanced recreation (fitness area, library, kitchen, computer lab, etc.) and community re-integration programming

clinical behaviour specialists, health care aids, licensed practical nurses (LPNs), occupational therapists (OTs), outreach workers, psychiatrists, physicians, protective service officers, recreation therapists (RTs), registered nurses (RNs), social workers
Eligibility

Service targeted to:

  • over the age of 18
  • primary diagnosis is psychiatric and stable
  • individuals on AISH or other similar funding
  • reside in the facility (6 months, up to 2 years)
  • requiring Level of Care Utilization System (LOCUS) level of care 5
  • would benefit from psychosocial rehabilitation, able and agreeable to participate in programming including: Cognitive Behavioural Therapy, CBSST, Independent Living Skills groups, etc
  • potential to benefit from recovery program including goal setting and attending programming to meet goals
  • able to engage in harm reduction if using substances or alcohol
