Alberta Healthy Living Program - Central Access and Triage

 Location: South Zone and Area

This is the Central Triage Department for the Alberta Healthy Living Program (AHLP) in the Southern Alberta. This central access referral service provides a single point of entry to those living with chronic conditions in communities throughout Southern Alberta.

The Alberta Healthy Living Program - South (AHLP South) is a chronic disease management program that supports people living with chronic diseases to improve their health, well-being and quality of life.  
This program provides services through group classes/workshops or individual consultations. Various health professionals are available to help support people. Services are provided either in person, online, or by phone. AHLP offerings vary across the province with provincial online education available to all Albertans.
Group workshops available either online or in person may include but is not limited to : 
General health: Stress, energy management, etc. 
Disease specific: Weight management, chronic pain, heart disease, diabetes, etc
Self-management workshops: Better Choices, Better Health® Better Choices Better Health

Specialty Care / Clinics may include:

Asthma / COPD Specialty Clinic: Individuals and families of all ages will work with a Respiratory Therapist to work on better managing asthma or Chronic Obstructive Pulmonary Disease (COPD). Assessment, treatment, and follow-ups are offered in individual appointments.

Diabetes Specialty Clinic: Provides one to one assessment and follow up for those living with Type 1 Diabetes, Type 2 Diabetes and / or Diabetes in pregnancy to individuals and families of all ages. Diabetes specialty also provides support and services to those requiring insulin pump therapy. Endocrine clinics with an internist or endocrinologist are available to pediatric patients, pregnancy patients, and those interested in insulin pump therapy. Nurse Prescribers are available to help support timely care. 
Cardiac Rehabilitation Program: Helps people recover after a cardiac event, heart surgery or cardiovascular disease diagnosis. The program provides education, support, and practical tools to help people manage their heart health and make healthy lifestyle changes. It includes individualized assessment, follow-up, classes, education, and exercise support.
Bariatric Specialty Clinic: This clinic provides assessment and support for patients undergoing bariatric surgery.  See the Bariatric Specialty Clinic
Clinical Exercise Therapy: Individuals referred by physician or health care providers will have a consultation with a Clinical Exercise Physiologist (CEP). The CEP will help to recommend what level of support best meets the needs of the client. Services may include assessment, clinical exercise therapy, education, and/or referral to alternative physical activity supports.
