Complex Care Program - Hospital

Provides medical care and case management of the most medically fragile children in Alberta Children Hospital's complex care catchment. Coordination of care during inpatient admissions and as patients transition from a prolonged hospital stay(s) back into the community.

The Alberta Children's Hospital Complex Care Program provides inpatient and ambulatory care and system navigation until care is fully transferred to a community pediatrician and medical home. For patients frequently re-admitted, the CCP serves as the primary medical home until stability allows for community transfer. The goal is to reduce hospital admissions, shorten stays, and improve family self-efficacy by ensuring consistent healthcare providers familiar with the patient’s history, facilitating care, knowledge translation, and comprehensive coordination across specialties.

Eligibility

Service targeted to child / youth up to 17 years old with:

  • Biomedical Complexity (chronic, fragile, multisystem co-morbidities)
  • Prolonged admission (>4 weeks) and / or recurrent hospitalization (>2 in 6 months), recurrent ED visits, lack of another medical home
  • Functional Limitation (multiple medical conditions accompanied by significant functional and / or adaptive impairment)
  • High Health Care Utilization across an ACH clinic(s) or program(s) (high service / resource utilization and service needs spanning multiple subspecialties and government ministries)
  • Patient and caregiver need - high intensity direct care requirements and associated impact on family functioning (e.g., financial, demands on time, appointments, and unmet care needs) and psychosocial / systemic complexity factors may be present

Referrals from pediatric and neonatal ICU, subspecialists, and pediatric hospital pediatricians. Self-referral is not accepted.
