Heart Transplant - Give Life Alberta - Transplant (South)

Provides multidisciplinary assessment, transplant eligibility evaluation, waitlist management, patient education, and lifelong follow-up for patients south of Red Deer being considered for or receiving organ transplantation. Services include pre-transplant assessment, ongoing monitoring while on the transplant waitlist, and post-transplant care delivered in collaboration with provincial transplant programs.

This service provides comprehensive transplant care, including:

• Assessment of transplant eligibility
• Education regarding advanced organ disease, transplantation, and self-management
• Pre-transplant diagnostic work-up and waitlist preparation
• Monitoring and management while awaiting transplantation
• Inpatient care, as required, during the waitlist period
• Post-transplant monitoring, treatment, and long-term follow-up
• Coordination of care with provincial transplant programs and pediatric transplant services, where applicable

Adult patients are followed at the Foothills Medical Center and pediatric patients are followed at the Alberta Children’s Hospital ensuring continuity of care before and after transplant surgery.
Transplant surgery is performed in Edmonton.
