Rheumatology Rehabilitation - Arthritis Care Team

The Arthritis Care Team is a specialized interdisciplinary team working with community partners that empowers people with rheumatological disease to self-manage based on shared decision making to optimize their health and quality of life.

The Arthritis Care team provides individual and group interventions aimed at

  • improving pain, stiffness and mobility impairments
  • managing or minimizing the risk of loss of function
  • optimizing participation and engagement in activities of daily living
  • improving wellness and quality of life
  • community reintegration

Rheumatoid Arthritis Group Management Program - "Managing Your RA"
Sessions include presentations, group participation and discussion, and individual treatment with an Arthritis Care Team physical therapist and occupational therapist.

  • joint management and joint protection
  • methods to relieve pain
  • fitness and exercise
  • managing fatigue
  • relaxation
  • nutrition
  • pharmacy management and medical update
  • strategies for living well
  • goal setting to achieve change

Ankylosing Spondylitis Group Management Program - "Managing Your AS"
Sessions include presentations, discussion and active participation
Learn about:

  • understanding ankylosing spondylitis and spondyloarthritis
  • managing pain and stiffness
  • fitness and exercise
  • tips for daily living
  • managing fatigue
  • medical update
  • goal setting to achieve change

Note

Rheumatoid Arthritis Group Management Program is;

Three consecutive days (Monday-Wednesday from 8:15 a.m. to 12:00 p.m. or 12:30 - 15:30)

Ankylosing Spondylitis Group Management Program is;

One half day (Friday from 8:15 a.m. to 12:15 p.m.)

Service Providers May Include
dietitian, occupational therapists (OTs), pharmacist, physical therapists (PTs), rheumatologists, social workers, therapy assistants
Eligibility

Must be under the care of a rheumatologist and be diagnosed with an inflammatory arthritis
