Acute Geriatric Medicine Service

Provides integrated acute inpatient, inpatient consultation, and emergency department geriatric medicine services for older adults, and selected younger adults whose needs are best met through an integrated geriatric medicine approach.

The Acute Geriatric Medicine Service (AGMS) is an integrated model of care that combines the principles of geriatric assessment with acute internal medicine management. Unlike services focused primarily on consultation or rehabilitation, AGMS provides active management of acute medical illness while simultaneously addressing fitness-frailty, multimorbidity, cognition, function, rehabilitation planning, medication optimization, goals of care, and discharge planning.

Eligibility

Eligibility is determined by clinical need rather than chronological age alone

  • Older adults, and selected younger adults, with acute medical illness
  • Patients commonly have multimorbidity, cognitive or functional impairment, or other complex health needs
