Seniors living in the community who have experienced a change in their functional status. This may include the following:

• Decrease in function, strength, balance, independence
• Recent injury or trauma
• Pain and discomfort
• Recent falls or increased risk of falling
• Recent discharge from hospital or emergency department visit

Referral can be from a physician, physical therapist or occupational therapist
