ACH Functional Rehabilitation Pathway

The functional rehabilitation pathway providers biopsychosocial multidisciplinary stepped care for youth and children struggling with functional symptoms who are actively followed by an Alberta Children's Hospital (ACH) physician.

The functional rehabilitation pathway providers biopsychosocial multidisciplinary stepped care for youth and children struggling with functional symptoms who are actively followed by an ACH physician in an ambulatory setting. Stepped care includes intake to ensure youth and families are ready to engage in rehabilitation services, a group education workshop, and 12 weeks of individualized coordinated multidisciplinary care focused on family-centered social, emotional, psychological and physical rehabilitation.
