Must have an abnormal neuroimaging or neurological / neuromotor exam.  Exam findings may include:

• Difficulty with walking / gait concerns
• Abnormal tone or posture
• Delayed motor milestones
• Possible underlying neurological or neuromuscular condition

Must have complex concerns related to motor abilities or motor development that have a functional impact on the child and can't be addressed by a community doctor

In addition to the motor concern, the child may also present with:

• Speech and language / communication concerns
• Cognition concerns
• Social/emotional concerns
• Adaptive / self-help concerns

The child must be referred by a community physician, pediatrician, or nurse practitioner for a specialist evaluation and recommendations. The referring provider must complete the referral form and a referral letter indicating medical needs and level of urgency and fax it to the service using the contact information in this profile.
