Helps people with diabetes prevent or delay diabetic nephropathy (kidney disease caused by diabetes).

This service:

  • assesses people with diabetes to help prevent or delay diabetic nephropathy
  • teaches about diabetic nephropathy
  • provides information about managing diabetes, hypertension, and cholesterol
