Community Rehabilitation Interdisciplinary Service

Provides virtual and in-person assessment and treatment for clients who would benefit from an active rehabilitation program with a focus on recovery of function and independence through client-centered goals, self-management skills and community reintegration.

This program provides rehabilitation services to adults living in the community who present with a functional impairment requiring an interdisciplinary approach to service. Conditions include:

  • Neurological conditions (such as Multiple Sclerosis, Parkinson Disease, Stroke, Acquired Brain Injury, Functional Neurological Disorders, Dementia, Post-Concussion Syndrome, and other neurodegenerative conditions)
  • Fibromyalgia, Chronic Pain
  • Post-COVID 19
  • Speech, language, fluency, and voice concerns
  • Other complex medical conditions with functional concerns across multiple domains
  • Lymphedema (Chronic, Non-Cancer Related - Mother Rosalie Health Services Centre)
  • Splinting (Strathcona Community Hospital)

Client goals may be addressed through self-management, 1:1, and group intervention. Education available for Energy Maximization for MS, Fibromyalgia, Chronic Pain, Swallowing, and Voice Care.

occupational therapists (OTs), physical therapists (PTs), recreation therapists, rehabilitation assistants, social workers, speech language pathologists (SLPs)
Eligibility

Clients referred to CRIS for rehabilitation must meet the following criteria:

  • 18+
  • has functional goals that are appropriate for a time limited outpatient rehabilitation program that cannot be addressed through other public resources
    • Requires interdisciplinary team approach; also accepts SLP or OT only referrals
    • Demonstrates “rehab readiness” and motivated to achieve rehabiliation goals
    • Medically stable, physically, and cognitively able to participate in an active rehabilitation program

Exclusion Criteria:

  • referral is related to a Worker’s Compensation Board injury claim
  • referral is related to a recent Motor Vehicle Accident
  • client is a resident of a care facility with access to rehabilitation services
  • client requires only physical therapy services. Call the Rehabiliation Advice Line for PT only referrals.
