Services target medically complex seniors who are at risk of losing their ability to manage independently in the community and who have a history of increased utilization of the health care system.The Carewest Comprehensive Community Care (C3) Program operates within two Calgary locations:
- Beddington C3 in northwest Calgary
- Sarcee C3 in the southwest
The C3 program is uniquely modelled to support vulnerable, older people residing in the community by providing timely, comprehensive medical care that is integrated seamlessly across a wide spectrum of services.
The major goals of the C3 Program (Comprehensive Community Care) include:
Supporting frail seniors living in the community to remain home safely for as long as possible by providing enhanced medical surveillance, maintenance rehab and socialization.
Further, the frequency of emergency visits and hospitalization for these seniors will decrease with the stabilization of their medical conditions.
