Comprehensive Community Care

Services target medically complex seniors who are at risk of losing their ability to manage independently in the community and who have a history of increased utilization of the health care system.

The Carewest Comprehensive Community Care (C3) Program operates within two Calgary locations: Beddington C3 in northeast Calgary, and Sarcee C3 in the southwest. The C3 program is uniquely modelled to support vulnerable and frail older persons residing in the community by providing timely, comprehensive medical care integrated seamlessly across a wide spectrum of services. In addition to the emotional, social, and recreational support provided by a traditional adult day program, C3's services also include complete provision of home care services, social work, occupational and physiotherapy, psychiatry and mental health support, and primary care provided by a family physician with additional training and certification in care of the elderly. Nursing and physician telephone support is available 24 / 7, 365 days a year. Clients who experience acute medical decompensation can be managed effectively at home, in the clinic, or in one of our overnight treatment beds located at our Sarcee location, potentially avoiding unnecessary visits to emergency departments and hospital.

Services may include:

  • bus service to the day centre 2 days a week
  • a recreation program/social program
  • a rehabilitation program
  • primary medical care provided by family physicans with extra training in Care of the Elderly
  • pharmacy services
  • lunch
  • respite care
  • short-term admissions for treatment
  • lab services

Service Providers May Include

Eligibility

For older adults living in the vicinity of Carewest Sarcee or Carewest Beddington:

  • have chronic, complex medical problems that may become unstable
  • have many visits to emergency departments or hospital units
  • need more support to stay living in the community
  • may present with signs and symptoms of cognitive impairment but their behavior will not interfere with their ability to interact with other clients
  • family requires support to continue in caring for their relative
Referral Needed

All admissions are through Community Care Access.

Service Access

A referral is required through Homecare. Once your referral is reviewed, you will be contacted to set up a provisional trial with transportation provided to and from the program. Home Care continues during this time. After this 2 week trial, you are admitted to the program or referred to another program that will be appropriate for your needs.

Fees

You have to pay a small monthly fee, which covers all costs except for the Alberta Blue Cross part of your medicines.

Fees will be reduced or waived based on client's annual income. No one will be denied access based on inability to pay.
