Comprehensive Community Care

Services target medically complex seniors who are at risk of losing their ability to manage independently in the community and who have a history of increased utilization of the health care system.

The Carewest Comprehensive Community Care (C3) Program operates within two Calgary locations:

  • Beddington C3 in northwest Calgary
  • Sarcee C3 in the southwest

The C3 program is uniquely modelled to support vulnerable, older people residing in the community by providing timely, comprehensive medical care that is integrated seamlessly across a wide spectrum of services.

The major goals of the C3 Program (Comprehensive Community Care) include:
Supporting frail seniors living in the community to remain home safely for as long as possible by providing enhanced medical surveillance, maintenance rehab and socialization.

Further, the frequency of emergency visits and hospitalization for these seniors will decrease with the stabilization of their medical conditions.

Service Providers May Include
doctors, licensed practical nurses (LPNs), registered nurses (RNs), occupational therapists (OTs), pharmacists, physical therapists (PTs), recreation therapists (RTs), social workers, healthcare aides
Eligibility
  • within C3 Beddington / C3 Sarcee Catchment area
  • aged 65 or older
  • assessed and approved for placement in LTC / SL (on waiting list), OR
  • has minimum of 6 medications and / or requires compliance packaging or currently has compliance packaging
  • meets the following RAI-HC criteria: Minimum of 3 on the MAPLe scale & Minimum of 3 on the DIVERT scale, AND

Requires therapies and / or day program as indicated by coding as limited assistance or more on at least 3 ADLs in Section H2 (ADL Self Performance) of the RAI-HC:

  • mobility: Transfers (Section H2b) and Locomotion (Section H2c and H2d)
  • dressing (Section H2e and H2f)
  • eating (Section H2g)
  • toileting (Section H2h)
  • bathing (Section H2j)
Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

A referral is required through Homecare. Once your referral is reviewed, you will be contacted to set up a provisional trial. AHS Home Care continues during this time. After the trial period (approximately 1 week), you are either admitted to the program or will continue with you current Home care services.

A client is not appropriate for C3 if:

  • cognition impedes participation in program
  • requires a transfer of 2PA (manual transfer) or mechanical lift
  • unwilling to attend day program or therapies on site
  • unwilling to transfer to Carewest physicians and pharmacy
Fees

There is a minimum monthly fee for the C3 program.

Medications costs are determined by our pharmacy and will be processed monthly.

Subsidy may be available to help cover the monthly fee for the C3 program based on eligibility. Coverage for medications will be dependent on Alberta Blue Cross and any additional benefits the client may have.
